Print the booklet yourself

Requirements:

• Double sided (duplex) printer
  
• Plain "Letter" sized paper (8.5" x 11")
  
• Stapler

* Please note that although detailed instructions are provided, any deviations from the original test booklet may result in inaccurate test results

Step 1: Download the test booklet, you can select either Version A or B, but not both.

Please open and download PDF file to your computer.

Step 2: Print the test booklet, please find instructions below.

The booklet must be printed 2 pages per sheet in reversed horizontal orientation and double-sided (long-edge binding).

Step 3: Fold and staple the booklet, please find instructions below.

The booklet must be stapled twice in the center and folded in half, please see below for visuals of the final product.
